Ecw social determinants of health screening

Are there any tools in ecw to screen for social determinants of health that is structured and trackable? We are currently working on setting up a patient kiosk and would like to incorporate screening done by patients in the waiting room.

At Neighborhood we created our own structured notes in eCW which are very easy to do and aggregate data daily to a org-wide dashboard. We have a SDOH program that consists of two types of assessments (high and low). Both have their own structured note so we can track SDOH for our general population as well as our complex patients. At CommuniCare we’ve implemented the PRAPARE Smart Form in eCW. One of the nice things about the Smart Form is that the responses pull into the Progress Note. And because it’s structure data we’re able to pull reports from it.

At Santa Rosa we’re using the same as CommuniCare. We have the PRAPARE smart form, but we’re also creating a paper version of the PRAPARE to hand to the patient so they can fill it out on their own. It’s the same words, just a different layout. So all the “yes” answers on one side and “no” answers on the other, to make it easier for the MA to see when something needs to be followed up on.
